Our Mission


The mission of C.A.R.E.S. is to assist individuals with developmental and intellectual disabilities and their families to obtain, identify, and maintain resources by:
Developing and implementing new programs and initiatives to improve the lives and independence of individuals with developmental or intellectual disabilities and their families.
Training community members to obtain the necessary knowledge and skills to assist individuals who have developmental or intellectual disabilities.
Helping families within underserved multilingual communities obtain access to information, resources and services.
Conducting ongoing research and evaluate the effectiveness of interventions to enhance program results.




Community and Confidentiality


C.A.R.E.S. employees have been trained to recognize the cultural needs of the various communities in which they serve. Confidentiality is carefully maintained and privacy is always respected.
Our earliest objective is to build a trusting relationship. 
As a program we are fully compliant with State, Federal and HIPAA regulations




Bilingual and Multicultural Services


Our bilingual and multicultural staff speak Chinese (Cantonese, Mandarin, and other dialects), Spanish, Japanese, Russian, French, Korean and other languages to help eligible individuals access and utilize needed services.




The Team


Our multidisciplinary team has extensive training and experience supporting individuals with Autism Spectrum Disorders (ASD), Intellectual Disabilities and their families. We use evidence based, data driven strategies to provide targeted behavioral interventions to enhance skills and achieve personal outcomes across multiple settings.

The Process of Getting an Evaluation


A referral is made to C.A.R.E.S.
Documents such as: Individuated Education Plan (IEP) and/ or previous evaluations are requested
A referral form is completed
Once the referral packet is completed and our clinical evaluation team reviews the documents, C.A.R.E.S will contact you for scheduling
Please note: parent or legal guardian must be present at the time of the evaluation.
Comprehensive Psychological & Psychosocial Evaluations
Our experienced and licensed Psychologists and Social Workers are trained to evaluate and diagnose individuals suspected of Autism Spectrum Disorders as well as, Intellectual Disorders. Clinical evaluations include parent interviews and all necessary testing. Current or previous Individualized Education Plans (IEP) and medical information is needed.
Evaluations are provided in convenient locations. Evaluations can be done in the family home, our offices, center or child's school. Parents need to ben present.
Transportation Reimbursement is Available for Eligible Individuals.




In-Home Respite Services


Respite care is designed to help families. Caretakers of children or adults with special needs often face serious problems and stress as a result of balancing the needs of their child or adult with special needs with the needs of other family members. Providers, chosen by the family, care for children or adults with special needs. These breaks, that respite care provides, allow families time to tend to the needs of their children, spouses, and themselves.
Respite care can be utilized on a regular or irregular basis and can be provided by family members, friends, skilled care providers or professionals




Residential Habilitation


This is a customized support service for individuals with intellectual and other developmental disabilities. We serve individuals living with their family or on their own in the community. The services are designed to meet the individual needs of the person within his/her community living environment.  Staff supports mainly focus on household management, mobility training, personal health care, medical appointments and social skills. 




Day Habilitation Program


Our program supports recent graduates and adults seeking to enhance their independence and community participation.
We use a person centered approach to address each individual's unique needs and preferences, support them to make choices and achieve personal goals.
Our clinicians focus on communication & social skills to encourage independence, facilitate community participation and self-advocacy.
Activities include planning & selecting options for recreation, employment and life skills, shopping, travel, budgeting, social skills, safety activities and wellness.




Prevocational Training


Our Prevocational Training Program supports recent Department of Education (DOE) graduates to enhance independence and develop employment opportunities in areas of personal choice.
Our job training curricula is designed and implemented based upon each individuals unique interests and abilities.
The hallmark of our program is individual choice. Individuals are choosing to learn diverse office skills, training to use computer hardware and software, scanners and other office equipment; design, build and market models and more.
We conduct detailed task analyses to develop prerequisite skills for employment such as the ability to communicate effectively in social and employment settings, identifying problems and problem solving skills, enhancing independence in self care & daily life. Our Prevocational Program also addresses social interactions, budgeting, personal care & physical activities.



  Medicaid Service Coordination  
  Service coordination assists individuals with intellectual and or developmental disabilities and their families in gaining access to services and supports appropriate to their needs. OPWDD delivers almost all service coordination through its Medicaid Service Coordination program (MSC). MSC is provided by qualified service coordinators and uses a person-centered planning process in developing, implementing, and maintaining an Individualized Service Plan. Plan of Care Support Services (PCSS) is an alternative form of service coordination. PCSS providers assist individuals to review and update their Individualized Service Plan. PCSS is delivered by a qualified Medicaid Service Coordinator.  


  Family Education Training  
  In all our programs we encourage parents to participate in their children's therapy and education. Parents are trained to support their children to maximize their potential.  


  Article 16 Clinic  
  Who is Eligible?
Any individual 3+ years old, with a previous diagnosis of a developmental disability and/or intellectual disability with the onset prior to age 22.

Mild, Moderate, Severe or Profound Intellectual Disabilities
Cerebral Palsy
Down Syndrome
Neurological Impairment.

What if my child does not have a diagnosis?
If you are concerned about your child's development, please contact our team with a current Individualized Education Plan (IEP) or prior evaluations.
What happens next?
The Clinic Treatment Coordinator reviews the referral for services and completes the intake process with the individual and/or their family. The final step is an eligibility review of the referral. If the individual is deemed eligible for services at our clinic the appropriate clinician will complete a needs assessment and develops a treatment plan to provide services as needed.
